D7241 Coding – Complete bony, with unusual surgical complications

Hey everyone,

I’ve been looking into the usage of D7241 (removal of impacted tooth, complete bony, with unusual surgical complications) and had a couple of questions regarding its application.

From what I understand, the code can be used when dealing with more complex surgical extractions, but I’m wondering, can D7241 ONLY be coded postoperatively if a nerve (IAN) is encountered or the sinus is repaired in the surgery?

Or, is it appropriate to code this preoperatively based on imaging?

Would appreciate any insights or experiences with how you’ve used this code and any advice on proper documentation to avoid disputes with insurers.
